I have been very impressed by the amount of time and effort the Social Services team (physios, OTs, SWs etc) have given to a friend of mine on her discharge from hospital/care home in order to allow her to go back to her own home. They have provided many aids and adaptations and done all that they can to make this possible. We shall see if it is successful. Well done.
